Hospice RN Case Manager

The registered nurse plans, organizes, and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Patient Care

Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).

Provides professional nursing care by utilizing all elements of the nursing process and as defined in the state Nurse Practice Act.

Assesses and evaluates patient's status by:

Writing and initiating plan of care

Regularly re-evaluating patient and family/caregiver needs

Initiates the plan of care and makes necessary revisions as patient status and needs change.

Uses health assessment data to determine nursing diagnosis.

Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.

Position Qualifications

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in-home care.

Current licensure in State and CPR certification.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Bachelor's degree, with one (1) year of hospice care experience preferred.

Management experience is not required. Responsible for supervising hospice aides.

Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:

Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

Assesses for muscle strength, mobility, gait, ROM-potential for rehab.

Directing physical therapy treatment.

Instructing patients and families/caregivers in the use and care of therapeutic appliances.

Determining priority needs for physical therapy.

Reporting to physician patient's reaction to treatment or changes in condition.

The speech-language pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.

Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

Identifies functional speech defects and establishes a plan of care to improve the patient's function.

Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

Recommends communication devices/aids as indicated.

Confers with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

We are seeking part-time In-Home Caregivers for the Day Shifts. We need caregivers who can be flexible and manage multiple shifts with different clients. Being a caregiver with Freudenthal is a definite calling that is very rewarding. In-home caregivers build lasting relationships and give assistance to people who are sick, injured, physically disabled, or the elderly and fragile. Caregivers work in a home setting and help their clients with activities of daily living, such as bathing and bathroom functions, feeding, grooming, taking medication, and housework, to name a few.
Caregivers help clients make and keep appointments with doctors, provide or arrange transportation, and serve as a companion for their clients. Caregivers maintain open communication between families and administration staff regarding the client's medical and emotional condition. Caregivers also document and report any changes in the client's health status, as well as ensure client safety and well-be. They are our front line of care at Freudenthal.

People Skills: Caregivers work in a very personal way with their clients. Some will be in pain or very sensitive to their fragility. Caregivers must be sensitive and compassionate with clients.

Schedule Driven: Caregivers are schedule keepers. Caregivers have to be there to make sure clients get up on time, make sure medication is taken on schedule and clients get to appointments on time.

Responsibility: Caregivers are trusted to provide the best care they can to their clients. In many instances, a client's well being and life may depend on their caregiver. It is for that reason that caregivers must be able to be trusted and must constantly demonstrate that they are capable, and responsible people. Throughout the duration of employment with Freudenthal Home Health, LLC, the Personal Care Assistant must meet the full job description as stated. If at any time during employment, you do not meet the full job description; your status may be changed to inactive until a physician's release is turned into the office.
